By ordering services from Nashville Placenta, I acknowledge that I have read, understood, and agree to the following:
I understand and acknowledge that, in accordance with applicable health and safety regulations, choosing to encapsulate or consume my placenta is a personal decision. It is not intended to diagnose, treat, or prevent any physical or mental diseases, ailments, or symptoms. I am choosing to consume my placenta based on my own personal, cultural, or spiritual beliefs.
I acknowledge that Nashville Placenta has provided me with clear information regarding both the potential benefits and risks of placenta consumption, and I have read and understood all relevant materials. I understand that my placenta will be prepared in accordance with OSHA guidelines and food safety protocols, and will be cleaned, cooked, dehydrated, and encapsulated in a sanitary, professional environment.
If my placenta is not encapsulated in my own home, I fully trust that it is being handled in a safe and clean setting. I confirm that I have provided documentation showing I have tested negative for communicable diseases, including STIs, and I understand that Nashville Placenta Services maintains client records for safety and compliance.
I release Nashville Placenta from liability once I have received my placenta products (capsules, tincture, smoothie, etc.), including any reactions or outcomes that may occur from use by myself or others. I also release Nashville Placenta Services from any liability in the event of transport delays or unforeseen circumstances outside of their control.
By placing an order with Nashville Placenta , I confirm that:
